What is Eyelid Surgery (Blepharoplasty)?

As we age, the skin loses its elasticity. Additionally, gravity causes excess skin to accumulate on both the upper and lower eyelids. On the lower eyelid, this excess skin can result in puffiness and wrinkles beneath the eyes, while on the upper eyelid, it may sag over the lashes, obstructing the field of vision. This not only detracts from one’s appearance but can also negatively impact eye health and vision.

Blepharoplasty is a surgical procedure in which excess skin and fat tissue are removed from the upper and lower eyelids. For lower eyelid surgery, the primary goal is to address under-eye bags. In contrast, upper eyelid blepharoplasty aims to tighten the sagging skin and restore the natural contour of the eyelid. It’s important to note that eyelid surgery is not designed to treat dark circles or crow’s feet.

How is Eyelid Surgery Performed?

Blepharoplasty is a surgical procedure that involves removing excess skin and fat from the upper and lower eyelids. The procedure typically takes about two hours and is commonly performed under local anesthesia.

The surgeon begins with the upper eyelid, making an incision along the natural crease. The skin is then separated from the underlying tissue, and excess fat and skin are removed. Afterward, the incisions are closed with fine sutures, which remain in place for 3 to 6 days, depending on the healing process.

For lower eyelid blepharoplasty, there are various techniques. In some cases, an incision is made inside the eyelid to remove excess fat, which eliminates external scarring. In other cases, an incision is made along the lower lash line to remove excess skin, tighten the underlying muscles, and remove fat deposits.

It is important to note that blepharoplasty cannot stop the natural aging process. However, the results can last from 5 to 7 years before further treatment may be considered.

Who is a Good Candidate for Eyelid Surgery?

The ideal candidates for eyelid surgery are individuals over 30 who do not have any underlying eye conditions. If you suffer from any of the following conditions, it is essential to consult with your doctor before opting for blepharoplasty:

  • Eye diseases such as glaucoma, dry eyes, or retinal issues
  • Graves’ disease or other thyroid disorders
  • Cardiovascular conditions or high blood pressure
  • Diabetes

Blepharoplasty is particularly effective for patients with sagging upper eyelids, especially if excess skin is obstructing vision. Individuals with under-eye bags or excess skin on the lower eyelids can also benefit from this procedure.

Before proceeding with surgery, patients must have a clear understanding of the expected results. While blepharoplasty significantly improves the appearance of the eyelids, it does not alter the overall facial structure. Therefore, it’s crucial to have an open discussion with the surgeon about your expectations and to set realistic goals for the results.

Post-Surgery Care: What to Expect After Eyelid Surgery

Blepharoplasty is a relatively minor procedure, and patients are generally discharged on the same day. Immediately after surgery, symptoms such as watery eyes, light sensitivity, double vision, bruising, swelling around the eyes, and mild discomfort may occur.

To ensure proper healing, follow these post-operative care recommendations:

  • Apply a cold compress to your eyes for 10 minutes every hour during the first night after surgery.
  • The following day, cool your eyes 4–5 times a day.
  • Use the prescribed eye drops and ointments regularly as directed by your doctor.
  • Rest and sleep with your head elevated for the first three days after surgery.
  • Wear sunglasses to protect your eyelids from sunlight and wind.
  • Avoid strenuous activities such as heavy lifting, swimming, running, or intense workouts for at least one week after surgery.
  • Avoid rubbing your eyes.
  • Bruising and swelling are common after blepharoplasty but usually resolve within 10 to 14 days.
